The Annandale & Eskdale Community Support Team is a community based person centred service working with a wide variety of support needs including mental health difficulties, learning disabilities, dementia and autistic spectrum disorders.

Our service uses person centred support planning to focus on your needs and what you want from your support. A key worker is allocated to you and we try to make sure these workers remain constant to give you consistent support. We will actively promote your independence and encourage you to be active in your community. Our team will respect your choices and decisions. We will support you at times that meet your needs and lifestyle. Your support can take place in your home or in everyday community settings. Support can range from personal care to social activities and emotional support.

Staff are always available to discuss your support and listen to you. We ensure that your support is regularly reviewed and things change when you need them to.
